What Is an Accident Report Form and How to Fill It?

The Accident Report Form is an official document completed by drivers to record the details of a traffic accident. Through this form, insurance companies can assess fault ratios and damage situations.

A correctly and fully completed form ensures that the insurance process progresses quickly and smoothly, preventing potential disputes.

When Is the Accident Report Form Used?

The form can be used if the following conditions are met:

  1. The accident resulted only in material damage
  2. No injuries or fatalities occurred
  3. The parties can agree on the situation
  4. Both drivers have valid driving licenses
  5. Compulsory traffic insurance policies are valid

If any of these conditions are not met, calling the police is mandatory.

Where Can You Obtain the Form?

Drivers are recommended to always keep a printed copy in their vehicles.

When Should It Be Completed?

3) Accident Time and Location

  1. Date
  2. Time
  3. Full address

4) Accident Sketch

This is one of the most important sections. The sketch should clearly show:

  1. Driving directions of vehicles
  2. Collision point
  3. Street names
  4. Traffic signs and lights

5) Marking the Accident Type

Relevant accident types on the form must be selected, such as:

  1. Lane change
  2. Entering main road from secondary road
  3. Rear-end collision
  4. Right or left turn

Accurate marking is essential.

6) Signatures of Both Parties

The form must be signed by both drivers. Without both signatures, it is not valid.

Important Points While Filling the Form

  1. Do not write statements admitting fault at the scene
  2. Avoid arguments or personal comments
  3. Do not mark options you are unsure about
  4. Check the form carefully before signing
